A 57-year-old black man with a history of type 2 diabetes, hypertension, dyslipidemia, and hepatitis C, presented to the emergency department with a 1-month history of right-sided chest pain associated with fever, cough, shortness of breath, and nausea.
Physical examination revealed a temperature of 101°F and decreased breath sounds on the right side. Complete blood cell count with differential revealed: leukocyte count, 17.2 x 109/L (normal, 4.0-11.0 x 109/L), without neutrophilia; hemoglobin, 11.2 g/dL (normal, 13.5-17.5 g/dL); hematocrit, 33.6% (normal, 42%-52%); platelet count, 483 x 109/L (normal, 140-400 x 109/L). The initial routine chest radiographs revealed the diagnosis (Figure).
